Chautauqua County Health Network Grant

The Silver Creek Central School District (SCCS) entered into an agreement of services with the Chautauqua County Health Network from Oct. 1, 2015, to Sept. 30, 2016.

The agreement is for assisting the CCHN in building and sustaining momentum around the Creating Healthy Schools and Communities (CHSC) initiative. Efforts will be focused on system and policy improvement activities that will be related to developing SCCS and the surrounding community into a place where it is easier to practice healthy behaviors. SCCS will assist with the integration of the school component of the CHSC work plan into its Innovate, Collaborate, and Educate 8 (ICE 8) Coordinated School Health (CSH) Initiative. SCCS will assist specifically in establishing school environments that support and promote increased opportunities for physical activity and healthful eating for all students in grades PK-12. SCCS will convene key stakeholders for the purposes of reviewing policies and investments toward Local Wellness, nutrition environments, and Comprehensive School Physical Activity Programming, and engage decision-makers to determine their readiness to undertake policy planning projects while building support for changes that will enable healthier behaviors throughout SCCS. This agreement may be extended and/or revised upon mutual agreement. Either party can terminate this agreement without cause upon 30 days’ notice with accounts to be adjusted and prorated as of the termination date.
